What is included in the monthly rent?
The monthly rent includes payment for all utilities (unless otherwise specified), three meals per day and snacks, housekeeping, laundry, cable television, assistance with all activities of daily living, medication management and coordination of medical care.
What insurance do you work with?
Acacia Heights primarily operates through private pay, however we assist with reimbursement from insurance plans specific for “Long Term Care.” We are NOT currently contracted with ALTCS, AHCCCS, Medicare or Medicaid.
Who will provide my day-to-day care?
Care is provided by state licensed caregivers who are overseen by an assisted living manager. BSN-prepared Registered Nurses will manage medications, coordinate medical appointments, and direct assistance with activities of daily living.
What is my assisted living Service Plan?
Upon move-in a plan of care, called a Service Plan, shall be developed by a Registered Nurse. The Service Plan specifies what type of care will be offered (incontinence care, transfer assistance feeding, etc), and personalized to each resident’s unique needs. Depending on the level of care required, the Service Plan shall be reviewed and updated every 3, 6, or 12 months, and with any changes in condition.
How do I get to my medical appointments?
We can arrange for a home visiting doctor to come see you right here at Acacia Heights, thus eliminating the challenge of arranging transportation to/from appointments. Otherwise, simply arrange with the manager to schedule a trip.
Can you accommodate my dietary needs?
Acacia Heights follows the “MyPlate for Older Adults” standard set by the Human Nutrition Research Center on Aging (see below). We also maintain a Low Sodium diet for all our residents. For our residents suffering with diabetic complications, we offer sugar free options. Please contact management directly for any other dietary requirements.
